Peripheral T/NK-cell lymphoma: a report of the IXth Workshop of the European Association for Haematopathology

Abstract

Aims:InApril1998,TheEuropeanAssociationforHaematopathologyorganizedtheIXthworkshoponperipheralT-cellandNK-celllymphomasandleukaemias.Theworkshopfocusedonunusualsubtypesoftheseraremalignancies,allowingevaluationoftherecentlypublishedWHOclassificationofneoplasticdiseasesofthelymphoidtissues.

Methodsandresults:One-hundredandthreecaseswerecentrallyimmunophenotypedandhybridizedforEBER1/2ofEpstein--Barrvirus.AllcaseswerereviewedbyapanelofexperiencedhaematopathologistsandclassifiedaccordingtothenewWHOclassificationforlymphoidneoplasms.ThreecaseswereconsideredasprecursorT-celland95casesasperipheralT/NK-celllymphoma/leukaemia.Althoughthecasesrepresentedaselectedseriesofunusualcases,thefollowingconclusionscouldbemade:(i)Mostlymphomasexceptthehepatosplenicgamma/deltaT-celllymphomasshowedaratherbroadmorphologicalspectrum,withdifferencesbothbetweenandwithinindividualtumours.(ii)Thisheterogeneitywasalsoreflectedbytheimmunophenotype,forinstanceavariableexpressionofCD30wasfoundinmanyenteropathytypeT-celllymphomas.(iii)Exceptionsinphenotypewereregularlyfoundinalmostallcategories,indicatingthatphenotypeshouldnotbethefinaldeterminingfactorinclassification.(iv)ThegreatmajorityofT-celllymphomasexpressedthealpha/betaT-cellreceptor,withtheexceptionofallbutonehepatosplenicT-celllymphomasandafewotherextranodalperipheralTcelllymphomas.(v)Malignanciesofprecursorcells,blasticNK-celllymphoma/leukaemia,adultT-celllymphoma/leukaemiaandmostAIL-typeT-celllymphomasdidnotexpresscytotoxicmoleculessuchasTIA1andgranzyme-B.Incontrast,allfiveaggressiveNK/T-celllymphomas/leukaemias,asinglecaseoflargegranularlymphocyteleukaemiaand40of47primaryextranodallymphoma/leukaemiasexpressedthesemolecules.Inhepatosplenicgamma/deltaT-celllymphoma,fiveofsixcasesshowedexpressionofTIA1butnotofgranzyme-B.(vi)Seventumoursdevelopedafterorgan-transplant,fourcasesbeingEBV-positive.Nodistinctphenotypecouldbeattributedtothesecases.

Conclusions:MostperipheralT/NKcelllymphomascouldbecategorizedasdistinctentitiesasdescribedintherecentlyproposedWHOclassificationforlymphoidneoplasms.
